​MINOR FORM INSTRUCTIONS

Any minor visiting Off the Trail Vacations Rentals & Campground without a parent or legal guardian present must have a completed Release, Waiver of Liability, Assumption of Risk and Indemnity Agreement / Adult Assumption of Responsibility for Minor Form completed by their parent or legal guardian.

​The adult accompanying the minor must also be listed as the authorized adult on the form.

​​Once the form has been submitted, you must verify your email to complete the process.
