MINOR FORMS INSTRUCTIONS

Any minor visiting Off the Trail Vacations Rentals & Campground without a parent or legal guardian present is required to have three separate forms completed by their parent or legal guardian.
 

All three forms must be fully completed for admission. The adult accompanying the minor must also be listed as the authorized adult on each form.
 

Once all three forms are submitted, you must verify your email to finalize the process.
